Overview:We are driven by the central purpose of our mission: “to improve the health status of the community.” As a Licensed Vocational Nurse (LVN)with the largest private employer in the Central Valley, the average day is anything but average and you will actively feel and see how your role impacts our mission and those we serve.The Community Subacute and Transitional Care Center (CSTCC) is a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision.

This position is located in our 48-bed short-term skilled unit, which provides wound care, IV therapy, and focuses on getting the patients transitioned back to their lives and community.

This unit houses a full rehabilitation gym for our patients.

Growth.Responsibilities:In this role some of your key responsibilities would include but are not limited to:Assesses, plans and implements the nursing care of residents within the skilled nursing setting.Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members.Participates in resident and family teaching.Maintains role as resident advocate with a focus on the facility’s mission.Qualifications:Experience and education minimum requirements:High School Diploma/GED.Fundamental acute-care nursing skills gained by completion of an accredited Vocational Nursing School program.License and certification minimum requirements:Current licensure as an LVN in the State of California.Satisfactory completion of an approved Pharmacology course is required if licensed by waiver.Current BLS certification.Preferred qualifications: Six months experience in a long term care facility.Click HERE to learn more about our awesome benefits offerings as well!
