The Community Subacute and Transitional Care Center (CSTCC) is a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision.

CSTCC is comprised of two unique and distinct units: Subacute and Skilling Nursing.

This position provides patient care in our 58-bed subacute unit, which is medically certified as a rehabilitation respiratory unit.

We specialize in trach weaning, helping patients come off of vents, feeding tubes, retraining patients to eat and breathe.Your Nursing Career at Community | Opportunity.

Responsibilities:The role of the clinical nurse is to deliver patient care through assessment, planning, implementation, and evaluation of patient needs with the highest professional standard while promoting nursing excellence through continued education and research.

This position treats patients in theSkilled Nursing unit of the Community Subacute Transitional Care Center.Qualifications:Experience and education minimum requirements:Graduate of approved nursing program.

Consistent demonstration of Novice / Advance Beginner clinical nursing practice proficiency.License and certification minimum requirements:Current California Registered Nurse License.
