You are at the center of the patient healthcare continuum, from admission to discharge.

You act as liaison to patient and family, physicians and external agencies, and the entire care team.

With a team of 150 Case Management team members the average Case Management nurse to patient ratios are 1:25.

A keen eye for detail comes naturally to you as does thorough and timely documentation and reporting.

You quickly grasp applicable regulations and standards, with the assurance to handle difficult situations on your own, or to escalate them as appropriate.
