Home Health-LVN

Valley Regional Home Health is looking for a kind and compassionate full time LVN to add to its growing Home Health team.

The LVN provides skilled nursing care under supervision of a registered nurse.

POSITION RESPONSIBILITIES:

• Assist the Registered Nurse in performing specialized procedures (as defined in the State Nursing Practice Act) as directed by the registered nurse under physicians orders.
• Assist with preparation, implementation and continuing evaluation of the patient plan of care.
• Actively participate in meetings to discuss patient’s plan of care is kept current and any identified issues are addressed.
• Help reinforce teaching to the patient/family members regarding end of life care.
• Foster cooperation effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
• Assume responsibility for the personal and vocational growth the development through active participation in nursing organizations and participating in in-services and other on the job learning.
• Prepare clinical and progress notes.
• Keep the Nurse Case Manager informed of changes noted during visits.

JOB REQUIREMENTS:

• Licensed LVN required.
• Ability to work well as a team.
• Excellent communication skills.
• Ability to work well independently.
