Preventative Medicine LPN/MA (Travel into clinics/homes/Work From Home) New Orleans Area

Affinity is currently looking for an LPN or Medical Assistant for Population Health to work from 8:00AM – 5:00PM, Monday through Friday.

This position will work from home and travel into member’s homes and primary care clinics in the New Orleans area with a Nurse Practitioner or PA.

Job Summary: The LPN/MA for Population Health will perform a wide variety of duties in the Population Health Program.

Primary concentration will be on patient interaction and assisting the Nurse Practitioner/Physician Assistant with Health Risk Assessment (HRA) visits in the home and/or provider offices.

The candidate must have strong phone communication and computer skills.

Essential Duties & Responsibilities:
• Gather patient information such as medical/surgical history, family history, risk factors, vital signs, and height/weight during the HRA visit and/or via telephone before the HRA visit.

• Assist with screening tests during the visit such as IRIS eye exam, PVD screening, HgbA1C, urine microalbumin, etc.

• Travel with providers in clinics and home visits to assist with triaging and EMR documentation
• Coordinate and schedule screenings for patients such as Mammogram, Colonoscopy, provider appointments, etc.

• Coordinate patient care with needed resources such as Integrated Care Management, Social Services, Diabetes care, etc.

• Assist with local Health Fairs
• Run Reports through proprietary software to help determine appointment needs when needed
• Responsible for scheduling and coordinating home visits and office visits when needed
• Coordinates care for SNP members between the Population Health department, SNP case managers, and any other affiliated resources.

• Assist with any new Population Health program initiatives Marginal Duties:
• Other duties as assigned Job Requirements: Knowledge, Skills, and Abilities:
• Valid LPN license OR medical clinic experience with triaging patients as a medical assistant
• Experience giving injections and taking vital signs
• Must possess and maintain a valid, unencumbered driver’s license
• Billing and claims knowledge are needed
• Strong computer skills
• Familiarity with Excel and Microsoft Word
• Familiarity with EMR
• Ability to function within several computer programs
• Excellent phone communication
• Communicates effectively with other employees and members
• Works independently and self-motivator
• Works in a timely manner
• Strong organizational skills and ability to multi-task
• Ability to interact effectively with all staff
• Strong verbal skills and grammar Supervisory Responsibilities:
• This position has no supervisory responsibilities Supervision Received:
• Works under general supervision of Population Health Supervisor and occasional supervision of the Population Health Manager Working Conditions:
• This position operates in a professional clinic environment and in homes of members
• Overnight travel/stays may be required
• This is a full-time position (40 hours/week)
• Overtime may be required
