Registered Nurse (RN) Hospice Case Manager

General Healthcare Resources is seeking qualified and compassionate RN Case Managers to join our team! Join an actively growing team that provides the highest quality palliative care services to patients and their families in the Fresno area.

Currently we have two openings for an RN Case Manager within this territory.

The Hospice Case Manager (RN) is a member of an interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional and spiritual needs of hospice patients and families. The Case Manager RN is designated to provide coordination of care, ensure continuous assessment of each patient’s and family’s needs and implementation of the interdis ciplinary plan of care.


· Schedule: Monday – Friday full time, make your own schedule!

· Caseload of 14-16 patients per week

· Mileage reimbursement (standard mileage rates)

· $30 monthly phone allowance

· Salary Range: $70,000-$83,000 (depending on experience) + competitive benefits package


· Completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement

· Completes documentation at the time of the event and at the location of care unless otherwise directed

· Provides education to patients, families, caregivers and referral sources

· Develops, coordinates, and follows the patient’s individualized plan of care

· Professionally manages/coordinates the hospice plan of care

· Provides and follows infection control practices


· Current and unencumbered RN license in the state of California.

· At least 1-2 years’ experience as a Registered Nurse, preferably in the home health or hospice setting

· Current CPR

· CA Drivers License

Job Requirements:

  • Integrate nursing case management with social work case management
  • Identifying patient/family care needs
  • Handle case intake and daily case management
  • Provide telephonic medical case management strategy
  • Serving in a case/care manager role
  • Designated as the case manager for hospice patients
  • Apply effective case management interventions
  • Perform initial case management assessment to determine care coordination and discharge planning needs
  • Evaluate outcomes of patient care
  • Assuming nursing case management responsibilities for designated patients
  • Identify members for case management
  • Provide case management services to geriatrics patients
  • Assume responsibility to coordinate patient care for assigned case load
  • Manage the patient case from pre-admission through discharge
  • Facilitating appropriate health care services throughout the continuum of care
  • Identifying appropriate patients for care management
  • Coordinate the overall interdisciplinary care plans for home health/hospice patients from admission to discharge
  • Evaluate outcomes of care with the interdisciplinary team and medical case managers
  • Providing patient case management services to assigned populations
  • Performing ongoing telephonic case management and treatment planning
