Essential Job Functions include those listed below.

Essential Job Functions:Case management of individuals who have been diagnosed with HIV and are currently affected by the COVID-19 virus.

Link consumers to community based providers to obtain supportive services.Make follow up contacts with Community Partners and consumers in the program to insure a smooth continuum of care;Heavy documentation;Coordination of services addressing all holistic approach to wellness and independent living.

Attend scheduled meetings within the community;Travel to community based providers;Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; andPerform any other duties as assigned.Working Conditions:Work is primarily performed in an office setting; Some travel is required; andSome outdoor activities may be required.Physical Demands:Occasional lifting of more than 10 pounds;Sitting, standing, walking, reaching are performed in the normal course of the position; andThe normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment.

Appropriate personal protective equipment will be readily available to every employee engaged in Category II tasks;QualificationsEssential Qualifications:Four to Five years case management experience;Prior experience working with the homeless, performing outreach, substance abuse and/or mental illnessExcellent communication skills;Basic computer skills;Good documentation skills;Be able to multi-task; andCPR Certification, First Aid Certification and an Annual Tuberculosis testKnowledge of HIV including transmission and care.Education:Graduation from an accredited four-year college with a Master’s degree in Psychology, Social Work, Rehabilitation Counseling or a related field is required.
