SUPPORTIVE HOSPICE CARE is a fast growing and expanding Hospice providing services in Orange county / Riverside County /Los Angeles counties currently seeking and hiring Registered Nurses preferably with Hospice Experience.

Must Have RN License, Current CPR, Must have RN License to practice professionally in the State, Must be accredited from a nursing School.

Responsibilities :Provide professional nursing care , complete admission, initial comprehensive, updated comprehensive, and ongoing assessments pf patientsInitiate, develop and update individual appropriate plan of care according to a individual patient’s needsCollaborate with physicians, and other team professionals including LVN, CHHA, Social Workers, Chaplains to provide careeducate patients and caregivers as appropriate .

Provides medication, diet and other instructions as ordered by the Physician and recognizes and utilizes opportunities for health counseling with patients and families / caregivers.

works closely with the Interdisciplinary Team.Frequently visits patients to assess their condition and evaluate POC, patient’s and PCG needsAttend IDG Meetings meetings as scheduledFollow the POC, Order DME as needed for patientSubmit all nurses notes on a timely manner into the system to be in compliance with HIPPA Regulations , maintain accurate recordsProvide nursing professional care, support and comfort according to individual patient s needsCollaborate with caregivers to plan and execute excellent patient careHelp in administering medication, order new medication or refillsAlleviate sudden symptoms of the disease s end phaseNotify and update a doctor for a patient s condition when deemed necessaryAssist patients and their families with paper work as needed with the help of MSW.Participate in bereavement program What is a Virtual Hiring Event?Virtual hiring events are a great way for employers and jobseekers to connect, even if they aren’t in the same physical location.
