Wellpath: One Care Manager Rn

Our talented, compassionate team thrives in an autonomous environment, is empowered and supported by education, training and the resources to practice healthcare the way it is meant to be and strives to ” Always Do The Right Thing!” Under direct supervision of the Health Services Administrator (HSA), Program Manager (PM), or higher medical professional, the Registered Nurse OneCare Manager will conduct and coordinate the patient plan of care through assessment, planning, implementation and evaluation in collaboration with the healthcare team and in compliance with all agency, local, state, federal laws and regulations.

Education for Registered Nurse: * Degree from an accredited school of nursing Experience for Registered Nurse: * At least two years’ experience as a Registered Nurse * Previous correctional/detention experience Licenses/Certifications for Registered Nurse: * Current (and must maintain) unrestricted Registered Nursing license, certification or registration in state of practice * Current (and must maintain) lifesaving BLS certification through AHA * Utilizes the nursing process to provide quality and efficient patient care according to established standards including: accurately completing assessments, and collecting, analyzing, and prioritizing assessment data to facilitate proper patient care * Collaborate and consult with the Discharge Planner, LMFT, LCSW or other Behavioral Health professional in the development of the participant’s individualized care plan * Attend treatment team meetings and share pertinent information with the other team members * Provide a nursing assessment of the medical and emotional status of patients and recommend treatment options * Implement the services detailed in the care plan * Monitor patients’ needs and provisions in the care plan * Arrange further medical/ psychiatric attention when necessary * Identify and develop support systems for the patient * Collaborate with PCP, Psychiatrist, and other health professionals to effectively manage patient health care needs using the lowest licensure level as is appropriate for the condition * Assist the patient in understanding the nature of emotional disturbances and accepting the need for treatment * Ensure patient record documentation meets program requirements * Maintain accurate records and documentation of patient treatments, immunizations, laboratory results and other information required for the patient registry/ OneCare reports * Maintain patient confidentiality in accordance with HIPPA regulations * Triage and communicate with physician, PA/NP, or Nursing supervisor any potentially significant medical complaints or observations discovered during routine duties * Maintain an active role in ensuring informed consent and capacity to consent is established, particularly for vulnerable groups and those with mental health problems or learning disabilities * Communicate effectively with a variety of individuals and function calmly in situations which require a high degree of sensitivity, tact, and diplomacy * Maintain a clean, safe, and secure working environment * May assist in the job shadow portion of the interview process, orientation and training of new and potential staff * May participate in performance evaluations and training of other team members * Report any abuse or maltreatment to safeguard the rights of detainees in accordance with mandated reporter’s and county, state, federal regulations * Perform other duties as assigned.
